Parent Consent Form

• I confirm that the information provided is true and complete.
• I understand that I must inform 3i Enrichment Centre of any changes to my child's medical condition or emergency contact details.
• I understand that my child should not attend class if he or she has a fever, vomiting, diarrhoea or a contagious illness.
• If my child requires urgent medical attention and I or my emergency contact cannot be reached within a reasonable time, I authorise 3i Enrichment Centre to seek appropriate emergency medical assistance. I understand that any medical costs remain my responsibility.
• I consent to 3i Enrichment Centre contacting me by phone, WhatsApp or email regarding my child's classes and centre-related matters.
• I understand that 3i Enrichment Centre takes reasonable care to provide a safe and supervised learning environment. I agree that children are expected to follow staff instructions and behave safely. Nothing in this form excludes any responsibility that cannot legally be excluded under Singapore law.
